Student Activity Form must be approved by the Student Council Representative and/or submitted to the Bookstore. Please do not submit a request if you do not have an approved Student Activity Form, we will not process this application until one is submitted and approved. This process is completed at the school level and then the Main Booster can move forward with processing any request submitted.
